Overview
Biohazardous waste is California's most tightly regulated everyday waste stream. The Medical Waste Management Act (Health and Safety Code section 117635) defines it as waste containing or contaminated with human blood or blood products, cultures and stocks of infectious agents, and related laboratory waste. In practice, that is your red-bag stream: blood-soaked gauze and dressings, contaminated gloves and PPE, IV tubing with visible blood, and specimen cultures. Used sharps (needles, syringes, lancets, scalpels) and pathological waste (recognizable human tissue from surgery or biopsy) are companion streams with their own container and treatment rules, and most facilities generate at least two of the three.
Generating any of it makes you a regulated medical waste generator, and California assigns you duties that never transfer to your hauler: register with CDPH or your county Local Enforcement Agency, keep waste in labeled, leak-resistant containers marked with the biohazard symbol, respect on-site storage limits (as short as seven days at room temperature for large quantity generators under section 118275), ship only with a registered transporter under a tracking manifest, and keep those records for three years. Pickup runs on direct routes covering all nine Bay Area counties plus Sacramento, Santa Cruz, and Mendocino counties, dispatched from our Fairfield headquarters. We are a CDPH-permitted medical waste transporter and a DTSC-registered hazardous waste transporter, the dual credential California requires to legally remove biohazardous waste from your site. Your driver manifests the waste at the dock, and the chain of custody stays unbroken from your storage area to final treatment. For low-volume generators outside our routes, our mail-back medical waste kits ship via USPS, and compliance consulting and training are available nationwide.
Treatment is where we differ from conventional haulers. Instead of incinerating the biohazard stream or autoclaving it into a landfill, we process it through the EnvoMed 80 system: the waste is shredded and sterilized to the STAATT Level IV microbial inactivation standard, and the cleaned plastic is recovered into virgin-grade recycling. The result is 100 percent landfill-free treatment, documented with a Certificate of Treatment and Recovery. Where California law requires incineration for specific streams, pathological waste and trace chemotherapy waste, we route those streams to licensed third-party facilities with full chain-of-custody documentation. Regulatory Framework
Biohazard waste disposal in California is governed by the strictest medical waste framework in the country. These are the rules that define what counts as biohazardous, how long you can store it, who may transport it, and how it must be treated.
Medical Waste Management Act

California's foundational medical waste law. It defines the regulated streams, sets generator duties for segregation, labeling, storage, and manifesting, and establishes the permit framework for transporters and treatment facilities. CDPH and county agencies administer it.

Defines biohazardous waste: waste containing or contaminated with human blood or blood products, cultures and stocks of infectious agents, and related laboratory waste. This definition determines what must go in the red-bag stream instead of the regular trash.
On-site storage limits

Large quantity generators (200 pounds or more per month) may hold untreated biohazardous waste above 0 degrees Celsius for no more than 7 days. Smaller generators get longer windows, and refrigeration extends them. Pickup schedules have to respect these limits.
Pathological waste treatment

Pathological waste, recognizable human tissue and body parts, must be incinerated in California and cannot go through alternative treatment. BAC routes this stream to licensed third-party incineration facilities under full chain of custody.

Penalty Warning
Under the Medical Waste Management Act, CDPH and county enforcement agencies can assess administrative and civil penalties of up to $10,000 per violation, counted for each day a violation continues. Knowingly treating or disposing of medical waste illegally is a crime punishable by fines up to $25,000, and OSHA serious violations for sharps and red-bag handling carry penalties up to $16,550 per citation.

Biohazard waste disposal is the regulated collection, transport, treatment, and documentation of waste contaminated with blood or other potentially infectious materials. In California the process is governed by the Medical Waste Management Act: waste is segregated into labeled containers at your facility, picked up by a CDPH-permitted transporter under a tracking manifest, treated at a permitted facility to destroy pathogens, and documented with records you keep for three years.
Any California business that generates waste containing blood or other potentially infectious materials: medical and dental offices, urgent care and surgery centers, dialysis and infusion clinics, skilled nursing facilities, veterinary practices, laboratories, and tattoo or body-art studios. If you generate biohazardous waste you are a regulated generator under the Medical Waste Management Act, regardless of size, and you need a permitted transporter to remove it.

There are no fuel surcharges, environmental fees, or per-container charges. No. California law prohibits putting untreated biohazardous waste, red bags, or used sharps into regular trash. Biohazardous waste must be treated at a permitted facility before disposal, and sharps must be in approved puncture-resistant containers, never loose in any trash stream. Violations draw penalties of up to $10,000 per day from CDPH or your county enforcement agency, and knowing illegal disposal is a criminal offense.
